Helping Care Teams Understand Patient Engagement – Theory and Practical Tools

This webinar will begin with reasons that patient engagement is so important. It will provide an overview of the theory that supports patient engagement and give insight into the current conversation around patient engagement. The second part of the webinar will show care teams and clinic leadership how to take advantage of the information, tools and resources on REACH’s new portal: Patient Engagement for Care Teams.

  • Understand the evidence that engaged patients can improve outcomes and reduce cost.
  • Learn about some of the leading thinkers in the patient engagement conversation.
  • Gain perspective on why patient engagement isn’t just an “e-thing”.
  • Learn how to use REACH’s new portal – Patient Engagement for Care Teams to find information, tools and resources.

Speaker

Phil Deering is a Regional Coordinator with the Regional Extension Center for Minnesota and North Dakota – REACH.  Prior to working for REACH, Phil spent 15 years assisting clients with major change initiatives, including enterprise software implementations, quality initiatives, and mergers and acquisitions. As Regional Coordinator, he directs a staff of consultants who assist providers at clinics and hospitals in reaching Meaningful Use. Phil has been active in developing tools and techniques to clarify client needs and deliver effective consulting services. His clients include integrated health delivery organizations, critical access hospitals and rural health centers, as well as community clinics and FQHCs.
